Counseling & Wellness is staffed with full-time licensed mental health professionals, and located at Oceanview Hall (next to the Campus Store) on 1000 S. Dixie Highway, West Palm Beach, FL 33401.

Requirements
All students attending our West Palm Beach campus must complete the health forms listed below. These forms should be submitted through MedProctor by August 1st for students beginning in the fall semester and by December 1st for those starting in the spring semester. Please to access and follow the instructions to submit all required health forms.
Graduate, Evening, Pharmacy, DNP, Part-time, and Ph.D. students
Full-time Undergraduate, International, and Additional Bachelors
- TB Risk Screening Electronic Form
- Immunization Form
- Physical Form (must be completed by a medical professional within the past 12 months)
- TB Clinical Assessment (only required if the student answered 鈥測es鈥� in any part of the TB Risk Screening Form)
